I installed fresh new TL 1.9.1 based on latest version of XAMPP, which include - Apache 2.2.6, - MySQL 5.0.67, - PHP 5.2.6.
The problem is in "Generate Test Specification Document" or "Test Report" page, etc, I can only see the line "Modify options for required structure of document...." and the check box, others can't be displayed. A yellow warning icon can be seen in the lower left corner of screen. Double click the warning, the detail info is like below:
