Test case result follow up

Is it a good thing to make a test case execution result follow up possebility

when a tester has execute a testcase there are follow ups on this result and for a good and complete documentation you want this information also on the testcase result sheet

anyone for this or is it already possible??

When a testcase is executed and didn't passed the test i like to follow the the information

a bug is made in the system or changes are made in the testcases. every step and action made before the testcase can be taken in retest i want to documentate. even if a testcase failure will not be solved in this build there must be an explenation in the testcase excution remarks.
